Actual Place of Diuretics in Hypertension Treatment-Juniper Publishers

Image
JUNIPER PUBLISHERS-OPEN ACCESS JOURNAL OF CARDIOLOGY & CARDIOVASCULAR THERAPY Abstract Diuretics represent a large and heterogeneous class of drugs, differing from each other by structure, site and mechanism of action. Diuretics are widely used, and have several indications in different cardiovascular disorders, particularly in hypertension and heart failure. Despite the large number of available anti-hypertensive drugs, diuretics remained a cornerstone of hypertension treatment. In the current editorial, we assessed the actual place of different diuretics in the hypertension guidelines focusing on the concept of tailored approach in prescribing them for hypertensive patients. Keywords: Diuretics; Hypertension; Hydrochlorothiazide; Indapamide; Guidelines Introduction Diuretics represent a large and heterogeneous class of drugs, differing from each other by structure, site and mechanism of action. Sudden Intraventricular Conduction System Defect Prior to Urgent Coronary Artery Revascularization-Juniper Publishers

Image
JUNIPER PUBLISHERS-OPEN ACCESS JOURNAL OF CARDIOLOGY & CARDIOVASCULAR THERAPY Abstract Intraventricular conduction abnormalities have been thoroughly described in the setting of post cardiac surgery in particular after valvular surgery. It’s also widely known that several anaesthetic factors are known to cause autonomic disturbances resulting in unmasking of a sinus node dysfunction, significant bradycardia, and cardiovascular collapse during the intraoperative period. However, little is known on asymptomatic episodes in particular prior to CABG. We report a rare occurrence of intraventricular conduction defect that presented in an asymptomatic patient following NSTEMI and prior to urgent coronary artery revascularization. We aim to highlight the measures that were taken to manage this. Keywords:  Conduction defect; Sick-sinus syndrome; Coronary revascularization Abbrevations:  NSTEMI: Non-ST Elevation Myocardial Infarction; ECG: Electrocardiogram; LCM: Le

Non-Invasive Hemodynamic Analyses to Guide Pharmacotherapy of High Blood Pressure: Mini-Review-Juniper Publishers

JUNIPER PUBLISHERS-OPEN ACCESS JOURNAL OF CARDIOLOGY & CARDIOVASCULAR THERAPY Abstract Various studies showed the possibility of the Risk Factors to promote a disease in the arterial wall and the following development and progression of the Atherosclerosis with its impact on future vascular events. The early stage of atherogenesis is the atherosclerotic plaque, and the primary-prevention had already proved itself efficacius in this early stage, because its erosion and rupture can lead to thrombus formation on the site of the plaque and vessel occlusion, culminating in Cardiovascular disease (CVD) with a variety of clinical manifestations. The most significant clinical manifestation, in terms of morbility and mortality, is CHD. The aim of the primary-prevention, dietary and lifestyle changes is the regression or slow-down and stability of atherosclerotic plaques, with the control of cardiovascular risk factors. Cardiovascular Risk Various studies showed

Vessel Architecture and Excessive Coronary Artery Instrumentation as a Cause of Coronary Artery Perforation-Juniper Publishers

Image
JUNIPER PUBLISHERS-OPEN ACCESS JOURNAL OF CARDIOLOGY & CARDIOVASCULAR THERAPY Abstract Coronary artery perforation (CP) is a rare but catastrophic complication of percutaneous coronary intervention (PCI). Grade three coronary artery perforation and rupture results in pericardial effusion and tamponade requiring urgent pericardiocentesis. Advances in coronary intervention have increased the opportunity to treat coronary artery perforation. We report a case of an elderly male patient with multiple cardiovascular risk factors presenting with Non-ST Elevation Myocardial Infarction (NSTEMI) complicated with pulmonary edema. Coronary angiogram (CAG) revealed severe triple vessel coronary artery disease (CAD) with heavy calcifications. Successful recanalization of mid left anterior descending coronary artery (LAD) chronic total occlusion (CTO) with heavy and floppy guidewires followed by stepwise rotational atherectomy (RA) with two different sizes burrs and high press

Heart Failure in Afro-Caribbean: A Cardiovascular Enigma-Juniper Publishers

Image
JUNIPER PUBLISHERS-OPEN ACCESS JOURNAL OF CARDIOLOGY & CARDIOVASCULAR THERAPY Introduction Heart failure is a disease in which there are striking population differences in almost every aspect of the disease. It has been recognized that the cause of heart failure is predominantly ischemic disease in nonblack but is related primarily to hypertension in blacks and this statement should be also assessed in Afro-Caribbean patients because population differences in disease exist that may be attributable to differences in social factors, genetics, environment, lifestyle, comorbidities, and complex interactions among these factors. The full impact of cardiovascular disease risk factors on incident disease and cardiovascular mortality in the Caribbean community is still not known, as there are not longitudinal studies on incident or fatal cardiovascular disease [1]. Data from St James Study in Trinidad showed that Blood Pressure, diabetes and low density lipoprotein w
