Variability in Antiplatelet Drugs' Response in Indian Patients Undergoing Percutaneous Coronary Intervention and Outcome Modification by Tailored Antiplatelet Therapy-Juniper Publishers

Image
JUNIPER PUBLISHERS - OPEN ACCESS JOURNAL OF CARDIOLOGY & CARDIOVASCULAR THERAPY Abstract Background: Double or triple antiplatelet therapy is a guideline mandated therapy for patients who undergo Percutaneous Coronary Intervention (PCI). However, there exists inter-individual variability in drug response to the antiplatelet agents leading to recurrent thrombotic events. We sought to study the efficacy and variability of thienopyridines (clopidogrel and prasugrel), ticagrelor and triple antiplatelet therapy in Post-PCI percutaneous coronary intervention (PCI) patients using AggreGuide A-100 platelet aggregometer, a point-of-care-test (POCT) device using laser light scattering technique. Methods: It was a single-centred prospective trial, included 405 patients who underwent PCI. Platelet reactivity was assessed by grading of platelet activity index (PAI) and considered as high on treatment platelet reactivity (HTPR) if PAI >5. Results: Patients on clopido...

Clinical and Hemodynamic Characteristics of Double Chambered Right Ventricle in Adult Patients-Juniper Publishers

Image
JUNIPER PUBLISHERS - OPEN ACCESS JOURNAL OF CARDIOLOGY & CARDIOVASCULAR THERAPY Abstract Introduction: Double chambered right ventricle (DCRV) is a rare congenital anomaly in which right ventricle (RV) is divided into two chambers by anomalous muscle band (AMB), a proximal high-pressure and distal low pressure chamber. Most of DCRV cases present during childhood, however, less commonly manifest in adulthood because of the nonspecific nature of symptoms leading to a delayed diagnosis. However, there are very few studies of DCRV in adults with regard to clinical features, hemodynamic data, treatment and prognosis, hence these are assessed in this current study. Methods: A total of 10 adult patients (age ≤18 years) diagnosed as DCRV by echocardiograpy and cardiac catheterization in our hospital from January 2008 to December 2015 were included. Nine patients had a history of a known heart disease since childhood (PM-VSD) with one patient being operated at the age ...

The Frequency of Adverse Effects and Early Outcomes of Cardiac Catheterization in Children-Juniper Publishers

JUNIPER PUBLISHERS - OPEN ACCESS JOURNAL OF CARDIOLOGY & CARDIOVASCULAR THERAPY   Abstract Diagnostic and interventional catheterization especially is invasive procedures that naturally can occur with risks and side effects for patients. This article examines the prevalence and detection of unpleasant side effects and early outcomes occurred deals by angiography. The study performed in patients undergoing angiography in Isfahan University of Medical Sciences in children department at one year. 372 patients, including 185 men and 187 women underwent angiography, which were 172 diagnostic angiography and 200 were interventional angiography. Among these patients, 105 had cyanotic heart disease, and 267 patients were non-cyanotic. Of the patients, 18 patients (5%) had unpleasant side effects or bad early outcome during angiography include (emboli of breaking wires and interventional devices in the body, death, drug allergies, fever and bleeding and need to blood transf...

The Frequency of Cardiac Arrhythmias in Children with Congenital Heart Disease during Angiography-Juniper Publishers

Image
JUNIPER PUBLISHERS-OPEN ACCESS JOURNAL OF CARDIOLOGY & CARDIOVASCULAR THERAPY Abstract Background: Although non-invasive methods in the diagnosis of congenital heart disease have a special and wide place but as a practical method, angiography used widely for diagnosis and therapeutic intervention in children. Because importance of arrhythmias and their risks during angiography, we decided to determine the prevalence and types of arrhythmias occurred during cardiac catheterization. Method: This cross-sectional study was performed in patients undergoing cardiac catheterization in Isfahan University of Medical Sciences during one year at pediatric ward of Chamran research and medical heart center. Results: From 372 patients (185 males and 187 females) who underwent angiography, 172 patients had diagnostic and 200had interventional catheterization of the patients, 160 (43%) had transient arrhythmias were less important during angiography and 15 (4.5%) were exp...

Tako-Tsubo Cardiomyopathy (Broken-Heart Syndrome)-Juniper Publishers

Image
JUNIPER PUBLISHERS-OPEN ACCESS JOURNAL OF CARDIOLOGY & CARDIOVASCULAR THERAPY Introduction In Japan as recently as 1991 TakoTsubo recognized as a syndrome, the physicians noted the characteristic changes in Left Ventricular apical ballooning similar to an octopus trap. Tako meaning Octopus and Tsubo meaning Japanese trap pot. It is also sinuously known as apical ballooning syndrome, acute stress-induced cardiomyopathy or commonly as broken heart syndrome (Figure 1). Etiology Caused by sudden surging stress hormones catecholamines (are epinephrine (adrenaline), nor-epinephrine (nor-adrenaline), and dopamine). Release of the hormones epinephrine and nor-epinephrine from the adrenal medulla of the adrenal glands is part of the fight-or-flight response for example, adrenaline essentially “stuns” or contracts the heart, triggering changes in heart muscle cells or coronary blood vessels (or both) that prevent the left ventricle from pumping effectively. Women ...
