We shall outline the contemporary history of the treatment of coronary heart disease, revascularisation guidelines and appropriateness criteria for different modalities of treatment, various randomised controlled trials (RCTs) and their meta-analyses comparing different methods of revascularisation for different categories of disease, the usefulness of multidisciplinary heart teams, the role of fractional flow reserve (FFR) estimation in guiding percutaneous interventions, the importance of surgical revascularisation without cardiopulmonary bypass, the parallel advances in percutaneous and surgical streams of treatment, risk prediction for the various modalities of treatment and informed consent.
