Combined Percutaneous Intervention of Coronary Artery Disease and Atrial Septal Defect-Juniper Publishers

JUNIPER PUBLISHERS - OPEN ACCESS JOURNAL OF CARDIOLOGY & CARDIOVASCULAR THERAPY   Abstract We report a case of successful combined percutaneous intervention in a middle aged female with coronary artery disease (CAD) and atrial septal defect (ASD). The case demonstrates the feasibility and safety of performing combined PTCA and ASD device closure in selected patients. Successful combined procedure obviates the need for major surgery and repeated hospitalizations and an effective alternative for patients at high surgical risk. JUNIPER PUBLISHERS - OPEN ACCESS JOURNAL OF CARDIOLOGY & CARDIOVASCULAR THERAPY   Abstract Complete occlusion of left main coronary artery (LMCA) is an unusual manifestation of coronary atheromatous disease. Most patients with chronic total occlusion of the LMCA presents with angina, some presents with myocardial infarction or congestive heart failure, but they rarely present with cardiac arrest and survive. Here, we report a rare case of chronic total occlusion of the LMCA who presented with cardiac arrest as a first manifestation of coronary artery disease. Our case is interesting as this patient didn't have any risk factors for coronary artery disease, had no angina symptoms before presentation, and had normal left ventricular systolic function. JUNIPER PUBLISHERS-OPEN ACCESS JOURNAL OF CARDIOLOGY & CARDIOVASCULAR THERAPY Abstract Intraventricular conduction abnormalities have been thoroughly described in the setting of post cardiac surgery in particular after valvular surgery. It’s also widely known that several anaesthetic factors are known to cause autonomic disturbances resulting in unmasking of a sinus node dysfunction, significant bradycardia, and cardiovascular collapse during the intraoperative period. However, little is known on asymptomatic episodes in particular prior to CABG. We report a rare occurrence of intraventricular conduction defect that presented in an asymptomatic patient following NSTEMI and prior to urgent coronary artery revascularization. We aim to highlight the measures that were taken to manage this.
