Does TAVI make you smarter? Exploring the effects of Transcatheter Aortic Valve Implantation on Cognitive Function-Juniper Publishers

Image
JUNIPER PUBLISHERS-OPEN ACCESS JOURNAL OF CARDIOLOGY & CARDIOVASCULAR THERAPY Abstract Background: Tran catheter aortic valve implantation (TAVI) is an alternative to surgical aortic valve replacement in high-risk patients. While early cognitive decline is commonly seen in patients undergoing cardiac surgery, the effect of TAVI on cognitive function is less well understood. The purpose of this study was to evaluate the effect of TAVI on cognitive function. Methods: A prospective cohort study, evaluating 91 patients undergoing TAVI between September 2010 and September 2014 was performed. Cognitive function was assessed using the Montreal Cognitive Assessment (MoCA) prior to TAVI and at 6 months follow-up. MoCA is a comprehensive test that assesses a variety of cognitive domains including visuospatial/executive, naming, memory, attention, language, abstraction and orientation. Results: Mean age was 79.2 years (SD ± 8.8) and median Society of Thoracic Surgeo

Cardiac Insufficiency: Understanding the Dimension of Advice-Juniper Publishers

JUNIPER PUBLISHERS-OPEN ACCESS JOURNAL OF CARDIOLOGY & CARDIOVASCULAR THERAPY Abstract Quality of life, in almost all the current literature reviewed; affirm that it is a set of conditions and situations that engulf the physical, spiritual, psychological, emotional well being, besides social relationships, family, friends, etc. If we analyze this affirmation, we can say that it is the daily life of every human being, healthy, productive, and active and that has internalized values like dignity and feelings of social belonging. Congestive heart failure is a condition in which the heart is unable to pump blood into the bloodstream in sufficient quantity to meet the needs of the body. (I would like to remind you that I am not a cardiologist and the point of analysis and revision in this article of opinion is about the quality of life and the emotional processes and impacts on the human being). In developed countries 6 to 10% of the population over 65 is affected

Clinical Outcomes after Intravascular Ultrasound Assessment of Coronary Intermediate Lesions-Juniper Publishers

Image
JUNIPER PUBLISHERS-OPEN ACCESS JOURNAL OF CARDIOLOGY & CARDIOVASCULAR THERAPY Abstract Purpose: Coronary angiography has limitations in the assessment of intermediate coronary lesions (ICL). Intravascular ultrasound (IVUS) can evaluate more accurately the severity of a lesion to guide the therapeutic strategy. This study sought to evaluate long-term clinical outcomes after IVUS-guided coronary revascularization of ICL lesions in patients from the Western Mediterranean region of coastal North Africa (Maghreb) in whom IVUS severity criteria have not been validated. Methods: We conducted a prospective monocentric study including 113 patients with angiographic ICL evaluated by IVUS. Minimal lumen area (MLA) cut-offs value to perform revascularization were 6 mm² for the left main coronary artery (LMCA) and 4 mm² for non- LMCA lesions. The primary outcome was defined as a composite of major adverse cardiac events (MACE) including death, myocardial infarction (MI

Role of Endovascular Recanalization and Stenting of Total Occlusions of The Renal Arteries For Blood Pressure Control in Resistant to Treatment Hypertension-Juniper Publishers

Image
JUNIPER PUBLISHERS-OPEN ACCESS JOURNAL OF CARDIOLOGY & CARDIOVASCULAR THERAPY Abstract Purpose: To assess t! he effect on blood pressure (BP) control and rennin levels of the percutaneous recanalization of totally occluded renal arteries in patients with resistant arterial hypertension, preserved blood flow in the sub-segmental renal arteries, and high level of plasma rennin. Methods: Between 2011 and 2015, we examined 7 patients with total occlusion of a Percutaneous recanalization was attempted in all of them. Success was achieved in 6 (85.7%) of the cases. All patients were hypertensive before the procedure, with mean BP values of 167.1/95.1 mmHg under systematic antihypertensive treatment with at least 3 antihypertensive agents. In all patients, plasma rennin activity levels were more than 2,5 ng/mL/h before the procedure. The patients had duplex signs of occluded renal artery and partially preserved subsegmental flow. Two of the patients showed normal s

Transthoracic Echocardiographic Guidance for Percutaneous Patent Ductus Arteriosus Closure in Pediatric Patients-Juniper Publishers

Image
JUNIPER PUBLISHERS-OPEN ACCESS JOURNAL OF CARDIOLOGY & CARDIOVASCULAR THERAPY Abstract Background:: Patent ductus arteriosus (PDA) is a common congenital cardiac disorder; the treatment of choice is fluoroscopy-guided transcatheter occlusion. Closure of PDA under transesophageal echocardiography (TEE) with Trans aortic imaging guidance was studied in adult patients. In pediatric patients, transthoracic echocardiography (TTE) provides excellent images for PDA and may replace fluoroscopy as an imaging tool to guide PDA closure. Purpose: The goal was to study the feasibility of device closure under TTE guidance in patients who are not candidates for transfer to the cardiac catheterization laboratory or in those with contraindications for contrast and/or radiation applications. Methods: Eighteen patients (7 males, median age 7 months, median weight 8 kg) underwent TTE guidance device closure of PDA from July 2013 to May 2015. Conscious sedation was used in 17
