In children, owing to poor tissue stores, and growing demand, a resurgence in carnitine levels may take much longer time. Intradialytic hypotension secondary to carnitine deficiency is commonly due to depressed cardiac function. During hemodialysis, both oral carnitine and intravenous carnitine has been equally effective in adults by reducing the signs and symptoms. However, in children, intravenous carnitine may be more effective in the management of intradialytic hypotension and improving heart function. However, several reports demonstrated that frequent PVCs were associated with left ventricular (LV) dysfunction, increased LV dimensions and cardiomyopathy. Case Report: A 52-year-old lady with underlying diabetes mellitus, hypertension and dyslipidemia presented with dyspnea and palpitation. Echocardiography showed ejection fraction (EF) of 25% with LV systolic diameter 5.2cm and mild functional mitral regurgitation (MR). Holter revealed 13657 isolated and couplet PVCs which represented 11.2% of total QRS count in 24 hours. Cardiac MRI showed no inducible ischemia, fibrosis, infarction or inflammation. Its efficacy appears to be implied by its proven ability to detect and defibrillate VT/VF. However the currently available data on the WCD is non-randomized and comes from several reports of single center experiences, and reviews of post market release, often vendor supported, registry data. In the absence of randomized trials, the effect of the WCD on patient survival cannot be determined. The mere ability to shock terminate VT/VF by the WCD does not suffice to assume survival benefit when used in large patient populations.
