Transcatheter valve-in-valve procedures represent a valuable treatment option in high-risk patients [ 1 , 2 ]. However incidental findings may contraindicate the transcatheter procedure. We describe the original case of a patient who was scheduled for transapical mitral valve-in-valve, but due to a left atrial mass underwent mini-invasive surgery with successful direct mitral valve-in-valve delivery and excision of the mass. Most of DCRV cases present during childhood, however, less commonly manifest in adulthood because of the nonspecific nature of symptoms leading to a delayed diagnosis. However, there are very few studies of DCRV in adults with regard to clinical features, hemodynamic data, treatment and prognosis, hence these are assessed in this current study. JUNIPER PUBLISHERS - OPEN ACCESS JOURNAL OF CARDIOLOGY & CARDIOVASCULAR THERAPY Abstract Acute pulmonary embolism (APE) is a cardiovascular emergency that without prompt clinical diagnosis and treatment may result in fatal outcomes. Indeed, the symptoms can be quite non-specific often causing clinical dilemma as well as potentially at risk of misdiagnosing on the initial presentation. More specifically, the electrocardiographic (ECG) manifestation of APE can be diverse from normal to sinus tachycardia with or without features of acute right heart strain. We present an unusual case of APE presented with the pattern of acute ST-elevation on the 12-lead ECG mimicking acute myocardial infarction; thereby discussing the clinical dilemma which arose with this case as well as all the potentially and relevant differential diagnosis for the presentation. Mitral Valve Replacement in Patients with Extreme Pulmonary Hypertension: Tertiary Center Experience of 680 Cases-Juniper Publishers

JUNIPER PUBLISHERS - OPEN ACCESS JOURNAL OF CARDIOLOGY & CARDIOVASCULAR THERAPY Abstract Many forms of left-sided heart disease can be associated with pulmonary hypertension (PH-LHD) which typically indicates more advanced heart disease, more severe symptoms and worse prognosis. Mitral valve disease, both insufficiency and stenosis, can lead to the downstream development of PH. Current guidelines suggest that the most effective therapy for severe mitral valve disease (stenosis and/or regurgitation) is surgical intervention, yet in some cases, the operative outcome bordered by high perioperative risks. Patients and methods: Data for this study included 680 cases of adult patients underwent first-time mitral valve replacement. All patients of the study were divided into 2 groups: those who had PASP 50-80mmHg categorized as severe pulmonary hypertension=(group I) and those who had PASP above 80mmHg Categorized as extreme PH=( group II).
